LIABILITY RELEASE:

In consideration of Gallatin Community Church allowing the above child to participate in Vacation Bible School activities. I, do hereby release, forever discharge, and agree to hold harmless GCC, its directors, staff, and volunteers (collectively herein the "Church") from any and all liability, claims or demands for accidental personal injury, sickness, or death, as well as property damage

and expenses, of any nature whatsoever which may be incurred by the signed name and the above child while involved in VBS

Medical Treatment Permission:

In the event of an emergency, I authorize GCC staff to take any steps necessary to obtain emergency medical care. I hereby release GCC staff from any financial liability incurred during such emergency treatment

Photo/Video Permission:

I DO give my consent to GCC to use photo or video images taken of my child in church brochures, advertisements for the church, or the website, in social media, and in other church publications as they see fit
